Description of Service Request

Please describe in detail the tenant's maintenance request in the field below. Please be sure to include when the problem started and provide as much detail about the problem as possible.

Entry To Your Unit

If you are in communication with the tenant, please ask them if we have permission to enter their unit at anytime during business hours to address the maintenance request. If so, please select the first option.

If you do not have explicit permission to enter the unit, please select the second option so a proper 24-hour Notice of Entry can be delivered to the tenant to make them aware of the service. If possible, please make the tenant aware that by not providing permission to enter, there will be delays in addressing their maintenance request.

Option 1 - I confirm that the tenant above allows permission to enter their unit at anytime during business hours (8:00am to 8:00pm) for the purpose of evaluating and completing the service request above, in the next 7 days.

Option 2 - The tenant does not (or has not) allowed permission to enter and will require a 24-hour notice of entry prior to entering the unit to complete the maintenance request.

Option 3 - This issue is located in the Common Areas and permission to enter the tenant's unit is not required.
